Pain on the top of the foot is becoming a more common problem seen in sports medicine practice. This pain on the top of the foot is more common in runners who forefoot strike first, especially barefoot and minimalist runners. There are many causes of pain on the top of the foot, but the most common in these types of runners is the higher forefoot dorsiflexion that occurs. This causes a dorsal jamming resulting in the pain on the top of the foot. The pain on the top of the foot occurs during running and pain can be palpated over the dorsum of the midfoot joints. The best treatment for this type of pain on top of the foot is to use foot orthotics to allow the foot to plantarflex and rearfoot strike.
